DDH/AM Review
Delivery Duty Holder/Accountable Manager (DDH/AM Review)
The DDH/AM will review and ‘sign off’ the report, they will ensure the investigation and recommendations are appropriate to the issues raised in the report and that any recommendations will address the findings. If satisfied the DDH/AM can close the report but should they decide there are wider implications beyond the DDH/AMs Area of Responsibility (AoR) or the DDH/AM believes it necessary the report may be submitted for ODH/AM Review. If the DDH/AM is not satisfied with the investigation the report can be returned to the Occurrence Manager for rework.
